Care shaped around you
Mental health support at home
Living with mental health needs can make everyday life feel harder. Our team offers steady, understanding support at home to help you manage day to day, build routines and feel more in control, at a pace that suits you.
Registered with the Care Quality Commission. Supporting adults aged 18 and over.

Why mental health is at the heart of what we do
Care Consort was founded by Khalil, whose background in mental health shaped the way we support people. That experience runs through our training and our approach, so support feels understanding from the very first visit.
“We see the person first, never the diagnosis. Everyone deserves support that meets them where they are.”
Who we support
- Adults aged 18 and over living with mental health needs such as anxiety or low mood.
- Younger adults who want support to manage a home and daily life with more confidence.
- People returning home after a hospital stay or a difficult period.
- Adults whose mental health affects their self-care, eating or daily routines.
- Families who are worried about someone and want reliable support in place.
We support adults aged 18 and over, and you are welcome to enquire for yourself or on behalf of someone you care about.

The support we provide
Your support is built around your goals and what helps you most. It can include:
- Building and keeping healthy daily routines
- Emotional support and someone to talk to
- Encouragement with self-care, meals and sleep routines
- Medication reminders
- Support to attend appointments with your GP, mental health team and other services
- Help with letters, bills and household admin
- Getting out, staying active and reconnecting with your community
- Working towards personal goals, step by step
Understanding, consistent and person centred
We see the person, not the diagnosis
You are more than a label, and your support reflects that.
Consistency matters
Familiar carers and predictable visit times help trust grow.
Your pace, your goals
We work towards what you want, in the time that feels right.
Culturally aware support
We respect how culture, faith and family shape how people experience and talk about mental health.
Working with others
With your permission, we liaise with your GP, care coordinator, social worker or family.
A trained team
Our carers are trained to support mental health with understanding and care.
We are here for support, not emergencies
Our support is not an emergency or crisis service. If you or someone you know is in immediate danger, call 999. For urgent mental health help, contact NHS 111 and choose the mental health option, or call the Samaritans free, any time, on 116 123.
